Probably not: Both the protected sex and the last test at >4 weeks from exposure would suggest against HIV infection. Nothing will make the chance absolute zero; that being said, your likelihood of having HIV with the scenario (and assuming, of course, properly done tests) is infintesmally low.

Not likely but yes: The actual time from the contraction of the virus until symptoms appear can take years, according to the center for disease control (cdc). Therefore, regular testing is recommended to avoid unintentionally spreading the virus.
