The simple answer is that it is the degree of aggressiveness (although it is more complicated than that). A choriocarcinoma from gestational trophoblastic disease is from being pregnant, so can't occur in virginal woman, but you can get choriocarcinoma not developing from gtd. Even a male can get this.
