See a CRS: Persistent or recurrent anal fissure with the correct anatomy (deep, chronic in appearance, with associated anal spasm and hypertrophy of the internal anal sphincter) respond nicely to surgery with an lateral internal sphinterotomy, see a board certified colon and rectal surgeon to see if you are a good candidate.

If the fissure is: Chronic most like it is, you will be best serfed by a sphincterotomy and fissurectomy done buy a colon and rectal surgeon. In addition to eating right, hi fiber, drink 80-100 ounces of caffeine free/alcohol free fluid daily.
