Pain not=survival: Pain is often not what determines if a condition is life threatening. Some of the soldiers and marines I have worked with reported excruciating pain after being impacted by ied's. I am told that passing a kidney stone or pancreatitis is very painful. Strangulated internal organs can hurt a great deal. I don't know what the most painful thing is.
