?nerve compression: Clearly, your description suggests for whatever reason, compression of greater occipital nerve. Could be due to cervical arthritis, posture, and even an early sign of neuropathy. But is there history of head or neck trauma which may have predated this? Might see your doctor and get this evaluated.
