It can: In women with large breasts there is increased stresses on the lumbar spine to stay upright and balanced. In some cases breast reduction has been effective at reducing the stresses and improving back pain.

Yes it may: Particularly large breasts may cause the shoulders to roll forward and cause excessive forward rotation of the upper back known as exaggerated thoracic kyphosis. It is quite common for some to report near immediate relief of back discomfort as soon as awakening in the recovery room although this is not typical of all patients.

Possibly.: Disproportionately large breasts may cause significant strain/pull on patients' necks and upper back areas. If this is the case, breast reduction surgery may be very helpful.
