Levofloxacin: These are side effects of levofloxacin. If you take an antacid it may interfere with absorption of the antibiotic. I suggest you take it at night after dinner, not on an empty stomach. If the problem continues, as your doctor to give you the 500mg dose for a longer period of time, you may tolerate it better.

See your doctor: You shouldn't require that much antibiotic for a uti. Make sure that there was culture and sensitivity done, so that the exact bacteria and treatment is identified. You may have symptoms that persist, but aren't associated with an infection. Excessive antibiotic treatment can lead to the symptoms you describe, among others. You may need to see a urologist.
