Timing: It is possible that the urine pregnancy test detected HCG that was in the urine even as a pregnancy was failing (impending miscarriage) but by the time a blood test was done, the miscarriage had already occurred and the levels of the HCG pregnancy hormone were cleared. Alternatively, it could have just been a false (+) urine pregnancy test.
