Not unusual, but: Botox might help, if not already used, and there is a surgery addressed over the Botox injection site outline. Occipital nerve blocks have been useful. Have you tried the best approaches statistically using otc preps such as butterbur, feverfew, riboflavin, coq-10, even magnesium. Lastly, a new dental device has been developed and may offer success.
