Did Fusion "take"?: Fused vertebra shouldn't really hurt. If you smoke your fusion could have failed. There may be hardware impinging on nerves or you could have muscle spasm or myofasciitis resulting from chronic muscle spasm. You might still have some stenosis or narrowing of the spinal canal causing pain. It sometimes takes months for a fusion to "take"-again smoking interferes with that process.

Chronic Pain: Unfortunately fusion surgery is not designed to treat pain. It is done to stabilize the broken pieces. The pain generators are plentiful. Each muscle, tendon, ligament, joint and nerve of each vertebral segment can generate pain patterns. After injury, arthritic changes above and below the fusion excellerate, and more pain can develope. Meds and injections can help a lot.
