For an ECG?: In the usual position. Ecg electrodes are placed based on the heart position and over certain ribs. Having implants is just having large breasts. Electrodes are not placed over the breast in women with large breasts.

Implant Electrodes?: Assuming you are talking about the electrodes placed on the skin to monitor the eletrical activity of the heart, the electrodes go in their usual places.

Normal places: Breast implants are inert, so electrodes may be placed in the normal location.
