Long answer: What autism is can't begin to be answered in a short answer. It is too complex, and involves many different things to diagnose. However, the answer to your other part of the question is a little easier. No, they don't realize hitting is wrong. They don't have "usual" concepts of right and wrong, almost no concept of appropriate social norms and how to interact with others.

Autism: This is a big question with big answers. Basically it is a developmental disorder that affects cognition and social development. And no, hitting is not typically about right or wrong but in reaction to smthg in their environment. Language delays affect verbal expression and can result in physical acting out.
