I'm not sure, but...: The mastoid bones are directly behind the ears and could become inflamed/infected from severe ear infections. Sometimes they even had to be removed, although this was much more of a problem in the past. There are also lymph nodes in the area behind ears and back of scalp. They can swell from infection, insect bites, and other causes. They could then cause pain.
