Consider fusion: If course of therapy , injections and facet rhizotomies have failed to relieve pain, surgery to fuse the spine may help. Replacement facet joints are still experimental but may help in the future. On some occasions spinal cord stimulation may help.

Hard to Say: Without evaluating you fully what to do. Recommend if you are not getting relief, seek another opinion. See a Pain/Spine specialist that is fellowship trained and has understanding of the musculoskeletal system like a physiatrist.
