None is the best tre: Steroids, symptomatic treatments have been tried. The response varies from patient to patient also the cause of arachnoid iris may be a factor, eg epidural, chemotherapy, trauma, infection etc.

No surgery: Arachnoiditis is a inflammation and scarring of the nerve roots and surrounding tissue (arachnoid) inside the dura. There are few treatments for this condition. Surgery creates more scarring. Pain management is the mainstay of therapy. Sometimes dorsal column stimuators can work quite effectively.
