Many things: There are many options including biliary disease. To rule out gallbladder etiology such as dyskinesia, you should have a hepatobiliary nuclear medicine scan with ejection fraction. If that's normal, you should see a gastroenterologist for possible endoscopy as well as your primary care for a chest x-ray as even pulmonary issues can present with abdominal complaints.

After eating?: You may have gallbladder dysfunction without having gallstones. If avoiding fat in your diet helps, it may be the gallbladder. If eating makes it worse... Then it may be the gallbladder. A scan we call the hida can demonstrate the function and test if the pain is coming from the gallbladder. May be worth pursuing.
