Bra investment: Reduction mammoplasty normally would not be covered by insurance just for cosmetic reasons & is expensive if you are paying out of pocket. Recommend investing in really good quality supportive bras that are specialty fitted for you. These can be purchased through specialty bra stores.

No: Breast lifts, and even breast reduction without symptoms expeected to improve with removing significant weight, are not normally covered by insurance. These are considered elective procedures that you would have to pay for yourself.

Possibly: Insurance does cover breast reduction surgery if you meet certain criteria based on height, weight, and anticipated amount of tissue to be removed. Best to have your surgeon get preauthorization to see if you qualify. If your breasts are not heavy enough, a skin reduction, or breast lift can be done to get you "perky", but this is usually an out of pocket expense.
