Mab boobs: About 1/2 of males go through some nipple or breast enlargement in puberty but this usually fades within a year or so.Persistence can require surgical removal of the breast tissue & fat that can persist. I would look into having this evaluated if it has persisted to age 21.

See doctor: If at the age of 21 you still have enlarging breast tissue then it's time for a medical/endocrine evaluation. Many causes of adult gynecomastia including obesity, hormonal abnormalities, certain medications, marijuana, etc.
