Many things: It could be many things. Arthritis or gout or infection are possible. If it is swollen, go to the rheumatologist to get it drained and sent for analysis. If you have fever and it is swollen, you should immediately go to the er to get it drained and be given antibiotics. Also, get an xray to see if it could be rheumatoid arthritis vs. Osteoarthritis.
