See a urologist!: The clots may periodically obstruct. Or the bleeding may be intermittent. Pain may relate to a stone fever to infection but cancer is a concern and needs evaluation now!

Many possibilities: Possible causes could be: urinary tract infection, kidney stones, bladder stone, renal infarct, tumor, prostatitis, or even "runner's hematuria." he should see his doctor to have a urinalysis and referral to a urologist. They may advise a ct scan or cystoscopy.
