See an orthopedist: You may have a subluxation of the head of the humerus. Simply said, your upper arm may be slipping in and out of place an orthopedist. He can certainly help.

Labral tear: It is possible you may have a labral tear. A good physical exam and possibly a arthrogram MRI could help diagnose what may be the cause. Arthritis is not likely at your age although there are some autoimmune causes that may be considered if you have problems with many joints. See a shoulder specialist.
