Many things : The ANA is a screening test for lupus and may be the only positive test in some individuals with the disease. However, the test is nonspecific and can be positive in numerous other diseases and occasionally in normal people without any disease. A positive ANA needs to be evaluated in the context of your symptoms. See a rheumatologist.

Need more testing: ANA is a screening test. A positive result only indicates more testing might be needed. The speckled portion of the result is not very helpful on its own. About 5% of humans (1 in 20) have a positive ANA for no reason.
