Need neurologic eval: It could some migraine or panic attack. However it could also be something very serious like intracranial bleed although you did not mention headaches. I would seek treatment asap.

Go to ER: Could be benign positional vertigo, but could also be several dangerous things, you need immediate evaluation.

Lots. CALL MD: Call your doc. May be anything from migraine to glaucoma, eye problems to to stroke to inner ear infection to low blood pressure to low blood sugar to high blood calcium to abnormal heart rhythms lots of other stuff. Don't know your age, medical conditions, etc. Call your doc now.
