Yes. Not common.: Yes, a heart attack in the 20's is possible. I have taken care of these. As a population of people having heart attacks, it is unusual, because it usually takes some time to develop the cholesterol in the arteries. This process is accelerated in patients with high cholesterol, high blood pressure, diabetes, smoking, or family history. Cocaine can also spasm arteries, causing a heart attack.
