Need careful exam: Most important, he needs a visit to a doctor (perhaps a cardiologist) for a detailed history, physical exam, appropriate blood tests, an ecg, and a urinalysis. The rest of the testing and therapy will depend upon his diagnoses.

Get Checked Out: He needs to see a doctor asap. These are serious symptoms. Could be heart, liver or kidney problems or a combination.
