Vital signs: If you are talking about general anesthesia, then we monitor blood pressure and heart rate. A rise in either usually indicates a need to deepen the anesthesia. If the patients vital signs do not respond to painful stimuli, it is generally assumed they are asleep. There also some brain wave monitors purport to measure level of consciousness.
