Depends on location: Upper back pain: could be vertebrae, ribs, or muscles such as trapezii, paraspinals or scalenes. Low back: could be scarring of dura mater (meninge) which attaches to head, neck and sacrum - or to other spinal segments if you had an injury such as an old disc hernia. Truthfully, there are too many possibilities to name, here. A good history & physical incl osteopathic hands-on exam may find/fix.
