Menstrual Malady: Depending on your age, the problem could be dysfunctional uterine bleeding, uterine fibroids, peri-menopausal bleeding or ectopic pregnancy. The best advice would be to seek and examination with a physician and possibly undergo an ultrasound. You could also conduct a simple pregnancy test to rule out a possible pregnancy. Only an ultrasound could determine if it's outside the uterus (ectopic).
