Foreskin was tight: This will happen again if foreskin does not retract easily. Suggest you use a lubricant for sex if your foreskin is loose. You could try using prescription strength steroid cream & streaching the foreskin if it is tight. Otherwise suggest you consult a local urologist with view to a possible circumcision which would take care of problem once and for all.
