A million things: You need a good clinician to take a full history - what kind of changes in bowel habits - constipation, diarrhea, maybe both alternating, and what kind of abdominal pain - sharp, dull, aching, does it change with activity or rolling over in bed. The most common cause of constipation with left lower quadrant pain is irritable bowel syndrome, but could be diverticulosis, appendicitis and many other.
