Paraneoplastic: To be paraneoplastic, a tumor has to be present. How do you or your doctor know that it is a paraneoplastic syndrome nd not caused by something else?

Plasma exchange. Yes: Treatment will probably work, but next step would be plasma exchange. You should consider having a pet / ct scan if not already done and repeat this in 3-6 months intervals out to about 4 years ( or 2 years in lems - your treatment with ivig suggests you have lems.).
