Noticeable pain only in left shoulder blade with some movement, but mostly when taking a deep breath. have been under much stress & smoking a lot.

Bursitis?: Possible subscapular bursitis. This bursa is between ribs and shoulder blade. Cushions when these two would rub against each other.
