Get treated.: Be certain you tell your prescriber that you are 22 wks in your pregnancy; this will enable them to select an antibiotic that carries little/no risk to the baby. It is important to get plenty of fluids. Keep well hydrated, as this reduces the risk of premature uterine contractions. It is also important to have a urine culture done at the end of the antibiotic treatment to be sure infection's gone.

Yes & no: Yes it could trigger premature labor so treatment is a must. But the germ will not be able to get to the baby. UTI's are a regional problem limited to your bladder and urinary tract. The uterus with its protective bag of waters protects baby from any intruders. Only infections that travel in the blood could get to baby & this does not.
