Yes: I would always consider extreme pain an emergency. It may well be caused by wisdom teeth. Perhaps there is something else going on. Please obtain a nice evaluation. There may well be an infection; you may need antibiotics. Good luck with that!

Yes and No: Wisdom teeth can definitely be a source of pain. However, the pain threshold for every person is not the same, and how we perceive and react to pain differs. Minor discomfort in one person may be extreme pain to another person. Pain in itself may not mean a true emergency, but pain with swelling and fever would suggest immediate evaluation and treatment.

Yes: Extreme pain can be suggestive of a possible infection. See a dentist or an oral surgeon asap. You can visit the er as a last option. If you rather have sedation for your comfort then consider visiting an oral surgeon.
