Because the muscle. : ...has separated leaving only some stretched fascia and the peritoneal lining holding Ur bowl in place. Any valsalva maneuver, in lifting or being upright makes the bowel want 2 leave Ur abdomen & this thin layer is stretched which is painful. U can get it fixed or wear an abdominal binder.
