Could be a fissure: Go see a colorectal surgeon or a general surgeon if a cr surgeon is not available to you. An anal fissure is a condition where the skin on the anal opening gets torn. Then the underlying muscle goes into spasm, causing pain. There are some non operative therapy which may help, sometimes an operation is needed. It is very fixable.
