Poor: The situation you describe sounds dire. His prognosis depends a large part on the cause of his heart failure and whether it could be reversible. Even if it is reversible, he sounds like he is in critical condition. I am sorry to hear this. I hope he gets better.

Not good: May need transplant if function does not improve on medications. Ventricular assist devices can be used as a bridge to transplant.
