Adductor strain : With the pain localized tithe groin and brought n with the position you place your leg in this could be related to the adductor muscle and a possible irritation to that muscle but i would also investigate the hip to see if this is an early form of a vascular necrosis of the hip.

Not sure: Changing your knee positions generally puts more stress on the nerves which emanate from your back. If you find certain positions are painful - do not do them. If you find you have a certain radiculopathy - (look this up) then mention it to your doctor on your next doctors visit.
