TMD?: Those symptoms sound more like tmd to me. You may want to have that checked by your dentist or someone who specializes in treatment of tmd (temperomandibular disorder).

Antibiotics used?: Difficult diagnosis. Would need to know what if any antibiotics you have been on and for how long. Are you running any temperature? Swollen glands in neck? X-ray normal to one doctor may not be normal to another and this should be reviewed by an endodontist (root canal specialist).

Very unlikely: Without a personal clinical exam and x-rays, we really can't diagnose your problem, although something like osteomyelitis would be very unlikely and should be easy to diagnose if present. You might want to consult with another dentist or even two.

Possible: You should see an oral surgeon at this point and a nuclear study should be done. But only if panoramic radiograph is abnormal. Any medical problems such as multiple sclerosis?

Possible: Without a clinical and x-ray exam, it is impossible to diagnose, but we all can add possibilities to a "differential diagnosis." whenever symptoms "don't make sense" or don't respond to routine treatment, cracked tooth syndrome is high on my list to evaluate. The x-rays will appear normal until time passes. Have your dentist examine the nerve chamber and roots with a microscope prior to removal.
