Nutritionist: If you have exhausted your options given by your doctor, i would ask if there is either a clinic nutritionist you could see or a pediatric nutritionist associated with either a hospital or another practice. It can be very difficult to find the right regimen to help with weight gain. Nutritionists can be very helpful in this regard.

I agree with Dr. D.: Pediatric dietitians have software that computes not only your daughter's caloric intake, but also %ages of each nutrient from a 3-day diet record. They work with pediatric endocrinologists, cardiologists & gastroenterologists, i respectfully recommend an evaluation by a "pedes gi" to find out if there is a medical cause of your daughter's undernutrition, even if her caloric intake is low.

Simple alternative: You can try Carnation breakfast essentials which comes in a variety of flavors.I find the nutrient contents similar and the cost less burdensome. If this doesn't help, the nutritionist idea is a good one.
