May be fungus: There is a list if the things it may be, but the most common is fungus. The second most common is eczema. Try a topical anti-fungal like lamisil (terbinafine). If no better then Cortaid 10. If it persists an exam and possibly a biopsy is warranted.

Hard to diagnose: Without more of a history and an examination. How long have you had this? Was there any injury or environmental exposure? Cortisone cream may help itching but see a foot specialist.
