Ankle: Sometimes avulsion fractures show up indefinitely on imaging. Check with your orthopedic surgeon for good exam if this is clinically significant or not.

No.: Chronic fractures ended up as nonunion. Which means the body has given up on healing and the fractured bone will just scar in. Depending on the fragment size it either needs to be removed or freshen up the edges and stabilized with screws if problematic. If not problematic then nothing needs to be done.
