Needs to be removed: I am not sure what you mean exactly but if your iud is falling out, you need it removed.

No sex if coming out: The iud might be in process of being expelled from uterus. Anyone with iud who can feel plastic in vagina should call the doctor who inserted it asap. Also she should avoid intercourse if the tip is protruding through cervix or if iud falls out. Btw male partner might notice this is happening before the woman does.
