Death vs Improvement: At 96 hip fractures and pulmonary emboli (blood clots in the lungs) are serious (not that they are ever not serious) and commonly lead to death. With exemplary care and a lot of luck she might be able to pull through, especially if her mental status is good. However you should be prepared for a fatal outcome, i'm sorry to say.
