No short answer: A number of possibillities, if he does much lifting he may be straining the muscles. He may have a prostate infection (that's often back pain though). He could have diverticulosis (pockets in the intestine) and they may get obstructed and/or infected at times(more often left side). Also chronic appendicitis (right sided) is a possibility although the medical community disagrees on of it occurs.
