For an explanation : Of events, ask whoever was designated as her next-of-kin, the person with whom your grandma's doctors could legally share medical information, to call risk-management at the facility in which she expired & request an explanation + a copy of her medical records. If she passed away at home, that person can request a copy of the autopsy report. Any dr. Would need to review records before responding.
