How many children die due to receiving a vaccine?

Almost None: According to world health organization so few deaths are attributed to vaccines it is difficult to assess statistically. Likely less than 1 in 1 million. A child is more likely to die from a vaccine preventable disease (or fireworks, a tsunami, an asteroid, a dog attack or one of many other rare things). http://www.who.int/immunization_safety/aefi/immunization_misconceptions/en/index4.html.

As low as 1 in 10 m: Estimates of random serious side effects range from 1 in a million to 1 child in 10, 000, 000. Although this is tragic, there is a greater chance of being struck by lightening, much less a significantly higher risk of the diseases themslves. Just ask the families of the dozen or so children who dies from whooping cough ( pertussis) in california in 2011.

Vs Lives saved?: Though a death from a vaccine is a tragedy, the greater tragedy would be the millions of people who would not be alive today if vaccines were absent from our formularies. For people who may have experienced a terrible side effect from a vaccine, they can report it to the "us vaccine adverse event reporting system" at http://vaers.Hhs.Gov/.
