A little: It difuses in all direction with gravity favoring some directions but only a little .Not clinically significant if a lower level is the suspected pain generator it should be injected directly the ole it will drip down and treat the next level as , a theory is diagnostically confusing and clinically un useful in my opinion.There is minor systemic spread via a small intravascular absorption.

Minimally: Patients are advised to limit activity for a day or so after a block..Feet up, resting comfortably, but steroids won't drift much because of gravity. No worries!

Possible: Most of the steroid stayed local where its injected, however like anything subject to gravity's pull the liquid can travel down as you stand up.
