Downturned mouth: The most common reason for a downturned mouth is a stroke, in which usually only one side of mouth droops and leaks saliva. Of course, chronic frowning will do it in patients who are unahppy. Some medications could do it by causing a dystonic reaction, which is usually temporary. I am not aware of any primary mental/psychiatric condition that does this. Very rare degenerative neuro diseases might.
