the national cancer institute (nci) has a a toxicity grading system used universally for clinical trials that grades from no or mild toxicity for each organ system (0-1) to severe (4) where medication is stopped.

the grading system is 1 to 5 (5 being death). It goes through all of the different organ systems and associated side effects.
