Side Effect Grades: This site explains what the grading system is used for in relation to HIV medications and their side effects. http://i-base.info/guides/side/how-side-effects-are-graded the national cancer institute (nci) has a a toxicity grading system used universally for clinical trials that grades from no or mild toxicity for each organ system (0-1) to severe (4) where medication is stopped.

CTCAE: Take a look at the following site which talks about common terminology criteria for adverse events: http://evs.Nci.Nih.Gov/ftp1/ctcae/ctcae_4.03_2010-06-14_quickreference_5x7.Pdf the grading system is 1 to 5 (5 being death). It goes through all of the different organ systems and associated side effects.
